Haemorrhoids (piles) are dilated anal cushions; they can bleed or prolapse. An anal fissure is a small tear that causes severe pain and streaks of blood on stool. A fistula is an abnormal tunnel after a drained abscess, often with persistent discharge. These are different conditions with different plans — a careful history and, when needed, examination under anaesthesia or imaging for complex fistulae.
First-line care
High-fibre diet, water, sitz baths, and medicines for stool softening help many fissures and early piles. Do not self-medicate steroids long term without advice. If bleeding is heavy, weight loss, or change in bowel habit occurs, do not assume piles — see a surgeon for a proper evaluation.
